Management of Breast Cancer in Older Women PDF-I was looking at Mrs T – all 45 kilos of her – with somewhat puzzled thoughts. I had prescribed her capecitabine at very prudent doses, in view of her 91-year-old kidneys and physiology. She had reduced my treatment even further, “because it was making her tired.” As a result, she was taking a grand total of 500 mg of capecitabine a day. Yet, her metastatic, ER/PR-negative, Her2-positive breast cancer was undoubtedly responding. Her pain was improving and her chest mass was shrinking, as were her lung metastases… What was the secret of that response? Were Mrs T’s kidneys eli- nating even less drug than predicted by her creatinine clearance? Was her sarcopenia altering drug distribution? Was she absorbing more drug than average? Or was her tumor exquisitely sensitive to fluoropyrimidines? “Physicians,” said Voltaire, “pour drugs they know little for diseases they know even less into patients they know no- ing about.” Medicine has made tremendous progress since the eighteenth century. Yet, there are fields where quite a lot remains to be learned. In developed countries, 25% of breast cancers occur in patients aged 75 years and older. Yet, these patients represent only 4% of the population of traditional clinical trials. That ought to let us wonder how relevant data acquired in patients in their 60s are to a nonagenarian. Fortunately, geriatric oncologists have been stepping up to the task and have gen- ated data to help us to treat such patients.

mele “Wonderful, excellent, precise, very happy, no problems for this item. Go
od conditions. Good bargain and I suggest to my friends.”

FEMME “Every day Millie cleans the lavish Winchester family home. She is in charge of picking up Nina’s daughter and cooking delicious meals for them before heading up to eat alone in her tiny room in the attic. Nina purposely makes a mess and tells Millie strange lies about her daughter. Millie feels bad for Nina’s handsome husband, Andrew, who seems like a broken man. Millie wishes she had Nina’s life. But something is strange, especially when she realizes that her bedroom door only locks from the outside. But rest assured, the Winchester family has no idea what’s coming.

THE HOUSEMAID is a story of a woman’s chance of redemption while struggling not to fall into the trap of her wealthy employer’s destruction. Narrated in the first person, Millie became a prominent figure I completely respected. Her witty personality shined through, as did her saint-like patience. Nina, the lady of the house, enraged me to my core. What a nightmarish boss to have, and don’t get me started on her bratty daughter. Despite the horrid treatment, Millie’s class and strength dealt with these dreadful personalities like a champ! As for Nina’s husband, Andrew, I found him to be quite dreamy and kind-hearted compared to the rest of his family.

Once I reached the second part, the twist hit me like a ton of bricks. Freida, you got me. Even though I didn’t see it coming, my heart sang with the juicy drama. Considering the spoilers, I can’t say much other than how revolting the Winchester family is. I cannot imagine anyone going through the craziness that ensued in this phenomenally written novel. By the end, everything wrapped up perfectly, and Freida officially won me over as a devote fan. I beg you to purchase THE HOUSEMAID, a true gem in the thriller genre. It will hit every element you look for while leaving you psychologically stunned and wanting more!”

V,.O ““If I leave this house, it will be in handcuffs. I should have run for it while I had the chance. Now my shot is gone.” – Prologue, ‘The Housemaid’ by Freida McFadde.

I combined reading with its unabridged audiobook edition, narrated by Lauryn Allman.

Right from the above opening lines I was hooked.
This is one of those domestic noirs/psychological thrillers that I feel is best read ‘cold’, so just the barest of details.

After the Prologue we go back three months earlier.
The housemaid of the title is Millie, who has applied for the position of live-in housekeeper for the wealthy Nina and Andrew Winchester. Millie is on parole and really needs this job. No further details though as might be expected from this type of thriller and the Prologue’s teaser, things get quite complicated.

Most of the novel focuses upon the dynamic between these three adults, though there is also the Winchester’s spoiled nine-year-old daughter, Cecelia, to add to Millie’s torments. At her first meeting with ‘CeCe’ she thinks “I’d say there’s at least a twenty-five percent chance she’s going to murder me in my sleep if I get this job. But I still want it.”

With respect to the audiobook, I felt that Lauryn Allman did an excellent job of capturing the characters, especially the two narrative points of view, Millie and Nina. Her voice is quite mellow and very easy to listen to. I have enjoyed her narration on two other recent audiobooks.

Overall, one of the best, twisty, unputdownable psychological thrillers that I have read for a while. After such a positive experience, I plan to check out Freida McFadden’s earlier novels as well as look out for her future projects.”
