I actually liked the DSM-4 version better. But this is still Great!

I love Gabbard! I cited his DSM-4 book heavily during my dissertation. In addition, I was asked to develop a program design for treating individuals with psychosis in rural Nevada. The chapter on Schizophrenia in his DSM-4 book was the most complete collection of clinical trials and literature that I have ever seen. There was 3-4 pages which were dedicated to using the common factors approach on what interventions were found to be helpful and what were found to be unhelpful. It was the greatest manual for treating psychosis !!! That chapter heavily informed my program design. Till this day the program is featuring great success and high numbers of recovery/remissions, and I very much thank Gabbard for that. However, I don’t understand why Gabbard took that common-factors part of his DSM-4 book out in this current DSM-5 version. The new chapter on schizophrenia and psychosis is somewhat abbreviated compared to his DSM-4 version. Furthermore, the wording left me believing that he now feels that psychosis cannot be treated effectively by psychodynamics. In 2020, a clinical trial found Mentalization Based Therapy (a psychodynamic approach) to be increasingly effective in treating psychosis, so I can’t understand why anyone would believe that psychodynamics cannot be effective. Furthermore, since this is a DSM-5 update, I wished he would have written more on PTSD and ADHD. I know that many in the psychodynamic world do not quite believe that these 2 disorders exist, as they believe that ADHD may be more of a defense mechanism linked to distraction against a core shame, but I wish he still accumulated the literature on this construct. I still feel that the DSM-4 version was superior, but I would still read this version as well. He does tend to integrate CBT into his new perspective, and he does write about the somewhat new-wave of evidence based approaches in psychodynamics such as mentalization and transference focused therapy. Lastly, I do feel that his chapter on narcissistic personalities is actually quite superior to his DSM-4 book.

Whether you’re taking a college course or not, it provides excellent information on psychodynamic therapy and its application toward disorders in the DSM-5.
