“Asymptomatic Atherosclerosis”, edited by Morteza Naghavi, is a comprehensive book assembled with the standpoint, that vascular death, heart attack and stroke should be prevented primarily with a new approach: rather than judging risk from traditional risk factors of atherosclerosis, prevention should be directed by the severity of atherosclerosis itself.

Recent research has documented that assessment of risk, based on risk factors of atherosclerosis, predicts very poorly who will suffer vascular death, myocardial infarction and stroke in the near future. In fact, using this approach for identifying the high risk person and offering intensive treatment has never been proven to reduce morbidity or mortality. Therefore, this book with a refreshing approach, where early (or advanced) signs of the disease (asymptomatic atherosclerosis), independent of risk factors of the disease, should be taken as a serious marker of risk, is very welcome.

The book therefore includes chapters dealing with pathogenesis, risk factors and new biomarkers of atherosclerotic cardiovascular disease. Especially, the potential of noninvasive imaging to disclose who are at the highest risk is covered in detail. Also, chapters dealing with how to use the biomarker and imaging information, including new treatment paradigms such as the “polypill”. In general, the authors of chapters are well-accredited scientists with decades of experience within their fields.

This book should be on the bookshelf for any physician who has an interest in atherosclerosis. It summarizes recent science with established knowledge and the only thing missing that comes to this reviewers mind, is that an “easy read” version for general practitioners would be needed when treatment of asymptomatic atherosclerosis evolves for the general population.

Asymptomatic Atherosclerosis, edited by Morteza Naghavi et al, provides the clinician with comprehensive state-of-the-art information regarding early detection and treatment of atherosclerotic cardiovascular disease.

The book starts with chapters which provide current knowledge regarding the pathophysiology of atherosclerosis, emphasizing points of vulnerability at various points in the cascade leading from micro to macro disease manifestations. There are very high quality color pathology pictures which help illustrate the important points. This is followed by several chapters dealing with methods of assessing risk, including biological markers, imaging of atherosclerosis, and functional assessment of the atherosclerotic process. The final chapters are concerned with current methods for treating asymptomatic atherosclerosis, including pharmacological, non-pharmacological, and invasive approaches for plaque stabilization.

This book includes in one source both the current knowledge and the potential future approaches to one of the most important challenges facing both the Internist and Cardiovascular specialist. The best way to impact on the future wave of disease is prevention, and this book provides a tool for understanding both where we are and where we are going. It will be useful for the practicing clinician who needs review of the current state of the art regarding prevention, and who want insight into future directions for both therapy and investigation. Conflicts of Interest and Departure from Evidence-Based Medicine

Readers and potential buyers of this volume should be aware of two things. First, the Society for Heart Attack Prevention and Eradication (SHAPE), which assembled this volume, and many of its individual contributors, appear to have extensive conflicts of interest in promoting these tests, as documented by the Center for Science in the Public Interest’s Integrity in Science (ISS) project (Google it). Indeed, “The SHAPE guideline appeared in a Pfizer-funded supplement to the July 2006 American Journal of Cardiology”.

Second: the purpose of the group and of this book is to promote is the use of coronary calcium screening to detect asymptomatic atherosclerosis. This notion was evaluated by the U.S. Preventive Services Task Force (USPSTF), the independent expert panel in prevention and evidence-based medicine mandated by Congress to evaluate the best scientific evidence on the potential preventive medicine technology and medical practice. After a rigorous analysis using the formal methods of evidence-based medicine, they determined that coronary calcium is ineffective as a test for the prediction of coronary heart disease, or of clinical events (such as heart attack and stroke) in *either* high-risk patients *or* apparently low-risk people (such as the detectioni of asymptomatic atherosclerosis for which SHAPE encourages it).
